This is a reference to the popular dystopian novel, The Handmaid's Tale, by Margaret Atwood. In this book, all women are forced into sexual slavery. Women are assigned to positions based on their value as breeders. A woman only has the value of her child.
Therefore, the strongest and most fertile women are placed at the top of the hierarchy. Women are not allowed to touch or speak to any man other than their master. They are not allowed to have any other children for fear that they will "infect" them with their inferiority.
If a woman does have children, she is punished by being sterilized so that she cannot pass on her inferiority genes to them.
